Keeping in Touch with You
The team at the ASCEND will stay in touch with you after your child leaves the hospital.
Before your child leaves the hospital, we ask you to complete the following electronic Contact Form. This Contact Form will gather information (like phone numbers and email), so we may reach you later and basic information about your household.
The ASCEND study team at your child’s hospital will help you complete this form.
If you need to update your contact information after your child leaves the hospital, please contact us by email at ascend-study@umich.edu or by phone at 1-735-845-5053.
