Follow-up Survey Information

The local ASCEND study team at your child’s hospital will help you complete the first two surveys. The study team will briefly explain how to answer the questions. You can answer the questions and return the surveys to the study team or the study team can help guide you through the questions as you complete the surveys.

The first survey will ask about your child’s overall health for the 7 days before their current illness. The second survey will ask about your child’s overall health for the 7 days before leaving the PICU.

If you are located in the United States:

About a month after your child leaves the PICU, the ASCEND study team will call you to complete the first follow up survey. You can complete the survey by phone with a member of the ASCEND study team. If you prefer, we can send you a link by email using a software called REDCap. You can use a computer, tablet, or smartphone to complete the emailed survey. In total, you will be asked to complete 4 surveys about how your child is doing.

If you are located outside the United States:

About a month after your child leaves the PICU, the ASCEND study team will contact you by email to complete the first follow up survey. We will send you a link by email using a software called REDCap. You can use a computer, tablet, or smartphone to complete the emailed survey. In total, you will be asked to complete 4 surveys about how your child is doing.
